How to Use Care Circle
The Care Circle allows you to include client personal contacts, medical contacts, and case managers in one place.
The Care Circle can be found in your client's profile.
There are four sections in Care Circle that will be reviewed individually.
1) Family & Other
2) Doctors
3) Staff
4) Staff Exclusions
Family & Other
This section will list any family members, guardians, or other trusted contacts.
To add a family member or other individual, click Add To Circle.
If you are adding an individual who has already been added, simply select their relationship, search their name and click Save. (For example, the son or daughter of a couple receiving care would be added as a New User once, then added again in the second profile.)
To add a New User, select the Add New User tab.
This will open the Add New External User window.
Complete the window with the contact's information.
Under User Type you will select Doctor, Contact, or Case Manager to categorize the type of contact of the person you are entering.
Note allows you to enter additional information.
Example of completed form:
Click Add New when finished.
You'll be directed back to the Add to Circle window and the Relationship will be pre-populated.
Begin typing the name of the individual you just added to the system under Person Name and select them from the dropdown menu.
In the case of Doctor, you will also further define their relationship by selecting one or all three of the options provided.
Click Save when complete and you will see the individual populate under the corresponding group on the client's Care Circle page.
Staff Section
The Staff section will automatically populate as you schedule your staff to client visits. In this area, you can remove the staff from the list by selecting the delete option or exclude the staff from the Staff Search when scheduling.
Staff Exclusions
This area will list any staff that has been excluded from scheduling.
